You can delete your slackr account yourself, from inside the app. There is no form to fill in and nobody to ask.
The same profile screen has Export my data, which downloads everything held for you as one JSON file. Do that before deleting — deletion is not reversible and no copy is kept for you.
Deletion starts immediately. Your sign-in is removed first, so you are locked out at once, and your records are then swept repeatedly over the following 75 minutes. The repeat sweeps exist because an access token issued before you asked can stay valid for up to an hour; they make sure nothing written in that window survives.
Two things outlive that window, and then go on their own:
If you have lost access to your account and cannot delete it yourself, write to privacy@slackr.pro from the email address on the account and ask for it to be deleted.
